Lisinopril is antihypertensive even in patients with low-renin hypertension. Hydrochlorothiazide : The mechanism of the antihypertensive effect of thiazides is unknown. Thiazides do not usually affect normal blood pressure. Hydrochlorothiazide is a diuretic and antihypertensive. Thiazides affect the renal tubular mechanisms of electrolyte reabsorption, directly increasing excretion of sodium and chloride in approximately equivalent amounts. Indirectly, the diuretic action of hydrochlorothiazide reduces plasma volume with consequent increase in plasma renin activity increase in aldosterone secretion, increases in urinary potassium loss and decrease in serum potassium.
