INTRODUCTION
TOPILIV 50 TABLET contains Topiramate, which belongs to the group of medicines called anticonvulsants. It is used in the treatment of seizure disorders in adults and children (over 6 years of age) alone, as well as in adults and children over 2 years of age. A seizure is a sudden, uncontrolled disturbance in the brain that can cause changes in your behaviour, movements, feelings, and consciousness level.
TOPILIV 50 TABLET is also used in the prevention of a type of one-sided headache called migraine headaches that causes severe throbbing pain or a pulsing sensation, nausea, vomiting, and extreme sensitivity to light and sound.
TOPILIV 50 TABLET should be taken with caution in patients with liver and kidney problems, a history of blood and body fluid abnormalities (metabolic acidosis), eye problems such as glaucoma (a condition that damages the optic nerve), growth problems, and patients who are on a high-fat diet (a ketogenic diet).
TOPILIV 50 TABLET is not recommended for use during pregnancy or breastfeeding or by women planning for pregnancy. TOPILIV 50 TABLET should be used with caution in children (2-16 years old) and the elderly.
The most common side effects of using TOPILIV 50 TABLET are stuffy, runny nose, or sore throat; tingling, pain, and/or numbness, sleepiness, tiredness, dizziness, nausea, diarrhoea, weight changes (weight loss or gain), and behavioural changes. Talk to your doctor if any of the symptoms get worse.

HOW TOPILIV 50 TABLET WORKS
TOPILIV 50 TABLET blocks sodium channels in the body that deliver electrical impulses to excitable cells such as nerve, muscle, and brain cells. This in turn enhances the activity of a chemical messenger in the brain to control the frequency of seizures. TOPILIV 50 TABLET also prevents your nerve cells from sending inappropriate signals in the brain, which could lead to migraine headaches.
DIRECTIONS FOR USE
Take TOPILIV 50 TABLET as advised by your physician. Swallow the medicine completely and do not crush or chew the medicine as it may leave a bitter taste. Drink a lot of fluids while taking TOPILIV 50 TABLET. Your doctor will decide the correct dose and duration for you depending on your age, body weight and disease condition.

Stop taking TOPILIV 50 TABLET and contact your doctor immediately if you experience any of the following side effects:
New or worsening signs of depression (persistent low mood, loss of interest in things, feelings of worthlessness and thoughts of harming yourself)

Seizures (fits)

Mood changes, anxiety, irritability, confusion, disorientation

Concentration problems, slow thinking, loss of memory

Signs of kidney stone (frequent or painful urination)

Signs of the increased acid level in the blood (troubled breathing including shortness of breath, loss of appetite, nausea, vomiting, excessive tiredness, and fast or uneven heartbeats)

Serious self-harming thoughts or trying to cause serious self-harm

Loss of part of the field of vision (difficulty finding or reaching for objects)

Signs of glaucoma blockage of fluid in the eye causing increased pressure in the eye, pain, or decreased vision

Signs of a high level of ammonia in the blood (changes in brain function leading to difficulty in thinking, remembering information, or solving problems, being less alert or aware, feeling very sleepy with low energy)

Signs of serious skin reactions, such as stevens-johnson syndrome and toxic epidermal necrolysis (rashes with or without blisters, skin irritation, sores or swelling in the mouth, throat, nose, eyes and around the genitals leading to peeling of the skin)

HOW TO MANAGE SIDE EFFECTS
Nausea or vomiting:
Take this medicine along with, or just after meals. Stick to simple meals and avoid eating oil-rich or spicy foods. If the symptom does not improve, contact your doctor.
Abdominal discomfort:
Avoid heavy meals and eat smaller meals at regular intervals. Drink a lot of fluids and go for short walks. If the symptom does not improve, contact your doctor.
Constipation:
Eat plenty of fiber-rich foods such as fresh fruits, vegetables, and cereals and drink plenty of water. Exercise more regularly. If the symptom does not improve, contact your doctor.
Stomach pain:

Dizziness:
Sit down or lie down slowly when you are feeling dizzy. Take rest for some time and drink plenty of water. Avoid coffee and other carbonated drinks. If the symptom does not improve, contact your doctor.
WARNING & PRECAUTIONS

PREGNANCY
TOPILIV 50 TABLET is not recommended for use in pregnant women or women planning for pregnancy as it may increase the risk of birth defects such as cleft lip (split in the top lip) and cleft palate (split in the roof of the mouth) and malformation of the penis (hypospadia). These defects can develop even early in pregnancy. Therefore, talk to your doctor about the best kind of contraception and consult whether TOPILIV 50 TABLET is suitable for you doctor before starting the medicine.
BREASTFEEDING
TOPILIV 50 TABLET is generally not recommended for use in breastfeeding women as it may pass through the breast milk and cause diarrhoea, sleepiness, irritability, and poor weight gain in infants. Therefore, you should consult your doctor before taking this medicine whether to abstain from breastfeeding or treatment with TOPILIV 50 TABLET.
DRIVING AND USING MACHINES
Do not drive or operate any machines after taking TOPILIV 50 TABLET, it might cause dizziness, tiredness, and vision problems. Consult your doctor before driving or using any tools or machines.
ALCOHOL
Avoid consumption of alcohol while taking TOPILIV 50 TABLET.
KIDNEY
TOPILIV 50 TABLET should be taken with caution in patients having kidney problems, especially kidney stones, or undergoing kidney dialysis. Consult your doctor before taking this medicine.
LIVER
TOPILIV 50 TABLET should be taken with caution in patients with liver problems. Consult your doctor before taking this medicine.
ALLERGY
Do not take TOPILIV 50 TABLET if you are allergic to topiramate.

Before taking TOPILIV 50 TABLET, inform your doctor if you:

Use in Pediatrics:

Medicines that impair or decrease your thinking, concentration, or muscle coordination [Ex. diazepam, clonazepam, alprazolam, triazolam, zolpidem]

Medicines that are used to prevent pregnancy [Ex. levonorgestrel, drospirenone / ethinyl estradiol, medroxyprogesterone]

Other medicines used in the treatment of epilepsy [Ex. risperidone, lithium, hydrochlorothiazide, metformin, pioglitazone, glibenclamide, amitriptyline,

Propranolol, diltiazem, venlafaxine, flunarizine]

St. John?s wort (hypericum perforatum) [herbal preparation used to treat depression)

Warfarin [medicine used to prevent the clotting of blood]
Overdosage:
If you or anyone else accidentally take too much of TOPILIV 50 TABLET, consult your doctor immediately or visit the nearby hospital. Symptoms of overdose are feeling sleepy, tired, or less alert, dizzy due to low blood pressure, feeling depressed or agitated, lack of coordination, difficulty in speaking or concentrating; blurred vision, abdominal pain, or seizures (fits).
