1. JAMB Result Document
2. Ordinary Level Result
3. Certificate of Local Government Origin
4. Two (2) Recent Passport Photographs
5. The photocopy of the Application Form and the Bursary receipt
6. Certificate of Birth
7. ND Outcome (HND Applicant)
8. Proof of Completion of One Year Information Technology (HND Applicant)
9. Documentation of JAMB Regularisation (HND Applicant)

Note: Nevertheless, sales of the form continue.
